Description :	A small, light-brown colored, square shaped piece of paper. The phone number of the business is in the top right corner of the page. Underneath the phone number, spanning the almost the entirety of the page horizontally, is a thin black line. Underneath the thin black line, on the left hand side, is the logo of the business. The logo is a medicine alchemist, with a black shaded circular background. To the right of the logo, are three lines going down the page indicating the name of the business: "D. Ross Cochrane, Prescription Specialists, The Rexall Store". Underneath the name of the business are three horizontally dotted lines where the clerk writes the date of the transaction and the name of the patron.
