Description : A birth certificate that has consists of a small laminated card with a white background and blue printing and decoration. The typed information is in black ink. The card has rounded corners. At the top of the card is a dark blue panel with words: 'Province of Ontario' in white capitals with a fine blue and white pattern border. At the top proper left corner is the Ontario Coat of Arms with the motto: 'Ut incepit Fidelis Sic permanet', meaning 'Loyal she began, loyal she remains'. Above the heading 'Birth Certificate' is a note: 'Form 26, The Vital Statistics Act 1948'. Below is the information name, date of birth, sex, place of birth, registration date and place and a number. The date of issue is the last item beside the register general's signature. The reverse has a red printed number that reads: '399693'.
Fonctions : Mary Thurston was born on August 29 1871 and married Joseph Featherstone in 1902. After Joseph passed away in 1913 Mary married Theodore Franklin Gould of Streetsville where she would spend the remainder of her life; she passed away on November 22nd 1958.
